Q: I have a 33-year-old twin sister who was recently diagnosed with clinical depression. Even though she’s been sad, she’s also worried, has restless nights, chest tightness, and racing thoughts. Our friends say it’s lack of faith. Our family doctor, who has known us since childhood, said it’s a “chemical imbalance” that medicine can fix. Is it faith or chemistry? And as her sister, will I be affected too?
A: Thank you for sharing this very important question. When we hear a story like this—a young woman struggling with sadness, worry, restless nights, tightness in her chest, and racing thoughts —we recognize it as a clinical syndrome. It’s not usually some kind of spiritual weakness. It’s not “just in her head.” And it’s almost certainly not a lack of faith.
What your sister is experiencing sounds like both depression and anxiety, two conditions that often show up together like twins—not identical but connected. Depression can make the world feel heavy and gray, while anxiety makes the heart and mind race with worry. Both are real, and both affect the whole person—body, mind, and spirit.
It’s Usually Not a Lack of Faith
There are cases in which weak faith leads to anxiety and despondency, leading some well-meaning people to say that if people only had stronger faith, we would never be depressed or anxious. They may quote Philippians 4:6, “Do not be anxious about anything” (NIV), and add Jesus’ counsel in Matthew 6:25: “Therefore I tell you, do not worry about your life” (NIV). But the full testimony of Scripture places these statements in their contexts. Consider Elijah, one of God’s great prophets, who once said he’d had enough and wanted to die. And David, who cried day and night, asking why his soul was so downcast. Even Jesus, in Gethsemane, admitted that His soul was overwhelmed with sorrow to the point of death.
These moments were experiences of being human. The brain, as with any other body part, can suffer an illness without that being a sign of little faith. The Bible never says that believers will never suffer emotionally. Instead, it shows us a God who meets people in their pain, not one who condemns them for it. Faith doesn’t always remove the storm, but it helps us hold on to the God of hope while the storm rages and passes.
So no, your sister’s struggle is probably not a sign of spiritual weakness, but even if it were, it certainly is an opportunity for the body of Christ—her family, her church, her community—to surround her with love, prayer, and practical care.
It’s More Than Chemistry, Too
Your family doctor isn’t wrong to say that brain chemistry plays a part, but he isn’t completely right, either. Depression and anxiety involve changes in how the brain’s chemical messengers— such as serotonin and cortisol—work. But decades of research have shown that no single “chemical imbalance” fully explains these conditions, and no single medication fixes everything.
The brain is much more like a living garden than a machine. Chemicals matter, but so do light, nourishment, and care. Medicine can help the soil—making it easier for therapy, community, and faith to take root—but it’s not the whole solution.
So What Causes Depression?
The answer is not simple. According to the most current information, we now see that depression and anxiety arise from many factors working together—some we can’t change, and others we can.
Things We Cannot Change
Genetics: Depression can run in families. Some genes make people more sensitive to stress or less able to regulate mood.
Gender: Women are nearly twice as likely to experience depression, partly because of hormones and social stress.
Medical or family history: Chronic illness or a family history of depression can increase risk.
Adverse childhood experiences: Abuse, neglect, or early trauma can shape how the brain handles stress in adulthood.
Past psychological trauma from abusive relationships, loss, war, or disasters, and chronic emotional or moral injury can produce wounds that increase the risk of anxiety and depression.
These are not signs of weakness. They’re part of a person’s story—and understanding them helps us respond with compassion, not blame.
Why Anxiety and Depression Often Travel Together
The sadness, worry, and restlessness you describe often appear side by side. That’s because anxiety and depression are “twin conditions.” Both come from the body’s stress system being overworked—the fight-or-flight response stuck in the “on” position. For some, that leads to worry and panic. For others, exhaustion and emptiness. Many people, as with your sister, feel both.
God meets us where we are, reminding us that our value is determined, not by how we feel, but by the reality of His love for us.
Her chest tightness and racing thoughts are not signs that she’s losing faith; they’re signals that her body and brain are crying out for rest and balance. God designed these systems to protect us—but when they never get to rest, they begin to hurt us instead. The good news is that they can reset with care, time, and compassion.
Will You Be Affected Too?
While the risk of depression in the general population is approximately 10 to 15 percent, the risk among siblings is 20 to 25 percent. As twins, you share about 40 percent of your genetic makeup related to mood regulation—so yes, there’s a higher chance you might also experience symptoms of depression or anxiety at some point. But that doesn’t mean you will; genetics are not destiny. What matters most is how you live, rest, connect, and believe.
Studies from the U.S. National Institute of Mental Health and others show that while genetics contribute to risk, environment, lifestyle, and faith have a stronger influence on whether those genes “turn on.” This is the science of epigenetics—how life experiences can change how our genes behave.
Meaningful work, regular movement, healthy food, and strong social or faith connections can actually protect against genetic vulnerability. Faith and purpose also buffer the brain from stress, reducing inflammation and calming the body’s alarm system.
So while you may share a biological sensitivity, you also share something much more powerful: love, empathy, and connection. You can support your sister by walking with her through treatment, interceding on her behalf in prayer, and taking care of your own well-being. Regular exercise, healthy eating, enough sleep, time outdoors, and spiritual fellowship all build resilience for both of you.
You can also gently remind her that getting help—whether through counseling, medical care, or prayer—isn’t a lack of faith. It’s an act of faith. It’s trusting that God works through doctors, therapists, family, and community to bring healing.
Genes may shape the outline, but God helps us color the picture.
The Whole-Person Answer
So is it faith or chemistry? The truth is—it’s both, and more.
Faith gives us hope and identity. Brain chemistry provides the physical foundation for thought and emotion. The two are woven together in ways we’re only beginning to understand. Depression and anxiety affect the whole person, and they heal best when we care for the whole person—body, mind, and spirit.
We are not just brains with chemicals, nor souls trapped in bodies. We are image bearers of God—fearfully and wonderfully made, with brains that can heal and hearts that can hope.
Cognitive behavioral therapy (CBT)—especially with a counselor who shares one’s faith—can be another powerful tool. It helps calm anxious thoughts and reshape thinking patterns, grounding healing in both truth and trust.
Taking prescribed medications under your doctor’s guidance is also an important part of recovery. Just as a plaster cast protects a broken bone, antidepressant medicine can protect a depressed mind while the body and spirit gradually recharge through rest, nutrition, and community.
Your sister’s story should be one, not of shame, but of restoration. And your love as her twin, your shared prayers, and your understanding may be part of the healing God is already beginning. Even if her faith was weak, God would provide the conditions for it to grow. The Scriptures are full of people whose faith began small but grew through God’s patient guidance and loving intervention. The stories of Abraham, Moses, Gideon, Peter, and Thomas show that God doesn’t shame weak faith—He meets it, tests it, and matures it through experience.
Faith and Science Agree
Both faith and science now tell the same truth in different languages: healing is possible. The brain can rewire. The body can recover. The spirit can be renewed. As Scripture says in Psalm 147:3: “He heals the brokenhearted and binds up their wounds” (NIV). That includes the wounds of the mind. God is still in the business of healing—through food, through medicine, through movement, through prayer, through love.
Throughout Scripture we see that God never abandons people in their despair. As with the examples above, Elijah wanted to give up, David wept through long nights, and even Jesus was overwhelmed in Gethsemane. Yet in each story God was there, providing what was needed—food, rest, comfort, and hope. And if anyone lacks faith, He creates the settings through which our faith can grow.
Genetics are not destiny. . . . Genes may shape the outline, but God helps us color the picture.
Depression and anxiety are no different today. God still meets us where we are, reminding us that our value is determined, not by how we feel, but by the reality of His love for us. Lamentations 3:22, 23, reminds us: “Because of the Lord’s great love we are not consumed, for His compassions never fail. They are new every morning; great is your faithfulness” (NIV).
Each morning we wake is proof that healing is still possible. So walk beside your sister gently. Be patient. Celebrate small victories. And know that even when her faith feels weak, God’s faithfulness is strong enough for both of you.
Steps Toward Healing—Things We Can Change
Exercise
Regular movement can boost mood as effectively as medicine for mild to moderate depression. Studies show that just 30 minutes of brisk walking most days can raise serotonin and endorphin levels—the brain’s “feel-good” chemicals.[1]
→ Caring for the body is a form of worship—honoring the One who made it.
Nutrition
A diet rich in whole foods, fruits, vegetables, legumes, and omega-3 fats supports brain health. The MIND diet, developed at Rush University, lowered depression risk by nearly 35 percent in one major study.
→ Feeding the body well helps feed the soul.
Sleep
Poor sleep triples the risk of depression.[2] Going to bed and waking up at consistent times helps the brain restore balance.
→ Rest is sacred; even God rested.
Vitamins, Nutrients, and Hormones
Low levels of vitamin B12, D, folate, ferritin, and thyroid hormones can mimic or worsen depression, especially in older adults, and should be checked. Supplementing B vitamins under medical guidance can improve energy and mood.[3]
Social Connection
Loneliness doubles the risk of depression. People who attend church, volunteer, or stay socially connected have much lower rates of depression and anxiety.[4] (Also, hugs help!)
→ God heals us through community.
Avoiding Toxins
Such substances as tobacco, alcohol, caffeine, marijuana, and other drugs can interfere with sleep, brain chemistry, and emotional balance. Even small daily exposures to pollutants or pesticides can affect mental health. Reducing these helps the body heal naturally.
Hydrothermal Therapy
Contrast showers or baths every morning seem to provide a temperature regulating positive stress or “body recharge” impacting on depression recovery.[5] Ask your doctor about potential contradictions for you personally regarding this type of heat-cold procedure.
Infections and Inflammation
After some viral or bacterial infections, lingering inflammation can disrupt mood-regulating chemicals. Managing stress, eating anti-inflammatory foods, and resting well all help the brain recover.
→ Healing the mind often begins with caring for the body—a temple of the Holy Spirit.
